Welcome to Park Meadows

Park Meadows is located just off I-75 near the Shands Hospital campus and the VA Medical Center. Park Meadows offers skilled and rehab services, private well-appointed suites, Alzheimer's care, and traditional long term care in separate and distinct units. As a resident at Park Meadows, you will receive individualized therapy throughout your stay. Every therapist at Park Meadows is experienced, certified and professionally licensed.

Our center's living spaces have been designed with our residents' function and comfort in mind. We have 154 well appointed suites in a home-like environment, including several large private suites. There are beautifully decorated common areas for our residents to share time with friends and families. Meals are served restaurant style in our elegant dining room.
